Paperless Billing Enrollment
Forms
Patients receive a paper statement by
default until enrolled in paperless billing. Patients who
sign up for paperless receive their billing statement
electronically to either a personal email address or text
message. A valid postal address is
required for the success of all delivery options. If the
electronic message cannot be sent, at the time of delivery,
a paper statement is mailed in its place. By providing your cell phone number, you are providing express consent to receive text messages regarding your account. Message and data rates may apply. Message frequency will vary by account and preferences.
Enroll in texting by clicking the link below and filling out and submitting our text form. This number must be associated with a phone that can receive a text message. Please allow up to, but not limited to, 1-3 business days for your account information to update.
Please use a private email address that only you, or someone authorized by you can gain access. It is not recommended to use a work, school, or organizational email address for paperless billing, your information could be exposed to administrators in those environments.
